My Log In Failed - What do I do?

If you enter the wrong username or password, you are unable to open Patient Services, Invalid username/password is displayed.

Retype your username and password ensuring you have the correct one and select Log in to retry.

If your number of log in attempts has reached the maximum of 5, your Patient Services account is locked for 60 minutes.

If you have forgotten your username and/or password, use the links on the Login page to request reminders.
